We are an assisted living facility located in Hennepin County, but we accept referrals from the 15-county metro areas, including assisted living facilities in Brooklyn Park, Minnesota. We serve people with mental illness and physical disabilities. We specialize in feeding tubes, diabetes management, oxygen management, chronic conditions that require one-on-one assistance, eating disorders, and behavioral management.
We accept referrals from all counties in the state of Minnesota, hospitals, and Intensive Residential Treatment facilities (IRTS). We accept CADI waivers, Brain Injury Waivers, Elderly Waivers, and private pays. We also accept Housing Support (formerly GRH) to assist people on waivers towards their room and board. We are not licensed for GRH only.
Our four 24-hour CL settings were developed and operated before January 11, 2021, and, therefore, are not limited to those 55 and older. We serve both males and females in co-ed or female-only homes in Brooklyn Park and Minneapolis.

We believe that every individual needs support from others and the community for them to continue thriving in their environment. We are here to give our support and provide housing stability to those who have been challenged with long-term homelessness and those who are discharged from hospitals, 90-day short-stay programs, nursing homes, rehabilitation centers, and prison.
